Active Cerebral Paragonimiasis Manifested as Intracerebral Hemorrhage with Pulmonary Paragonimiasis

A 46-year-old man was admitted with left sided weakness and dull headache with vomiting. The patient had first been seen at our hospital three months earlier because of intracerebral hemorrhage. He had pulmonary Tuberculsis and had been treated with anti-tuberculosis medications during the last 4 months, but blood tinged sputum persisted. Brain MRI showed conglomerated multiple irregular ring enhancing nodules in the right periventricular white matter in both the T1- & T2-weighted images. In the blood laboratory results, a proportion of eosinophiles increased as much as 13%. Results of enzyme-linked immunosorbent essay showed positive reaction for Paragonimus specific antibody in both serum and cerebrospinal fluid. After the patient was treated with praziquantel, hemoptysis and brain lesion subsided. We suggest of cerebral paragonimiasis as the cause of intracerebral hemorrhage with history of hemoptysis and history of eating freshwater living things even though cerebral paragonimiasis is a rare cause of intracerebral hemorrhage.
